The deadline to complete

Step One (Biometric Testing) and

Step Two (Health Risk Assessment Survey)

for your Wellness Program participation

is SEPTEMBER 30th!

Failure to comply will result in your Wellness Discount being removed.

All employees who are removed from the Wellness Program this year will not be eligible for the program next year.

If your spouse is covered on your plan, he/she must also comply.
