New Hires

Those hired 2/1/10 or later can wait until onsite biometric testing in the Fall to complete these steps.

Steps 1 & 2 (Biometric Testing and HRA) must be completed within 60 days of your benefits effective date.

Step 3 (Annual Physical) is due DECEMBER 31, 2010.

All Participants

Annual Physical forms for the 2010 plan year are due 12/31/10. 

Onsite Biometric Testing and Health Assessments will be in the Fall for the 2011 plan year.
