New Hires

Those hired 2/1/10 or later can wait until onsite biometric testing in the Fall to complete these steps.

Steps 1 & 2 (Biometric Testing and HRA) must be completed within 60 days of your benefits effective date.

Step 3 (Annual Physical) is due DECEMBER 31, 2010.

Wellness Plan Requirements

If you do not complete these steps you will be removed from the Wellness Program, lose your discount, and will not be eligible to participate in the following plan year.

Step 1: Biometric Testing
Download the Biometric Testing Form

 

- A simple and quick test to get your height, weight, blood pressure and finger-stick for bloodwork.
- All Wellness participants need to complete this step.  This means you and your spouse if enrolled on the plan.
- Open Enrollment renewal biometrics will be onsite.  New Hires throughout the year will need to make an appointment with their physician and get referred to a lab.


Step 2: Health Risk Assessment (HRA)

- Go to www.myuhc.com to answer simple questions about your health and medical history.
- All Wellness participants need to complete this step.  This means you and your spouse if enrolled on the plan.  (Your spouse needs to register their own user name in order to complete their HRA.)
- Once you have completed the HRA, print off your confirmation page and save for your records.


Step 3:  Annual Physical

Download the Annual Physical Form

- Schedule your physical with your doctor before DECEMBER 31, 2010.
- All Wellness participants need to complete this step.  This means you and your spouse if enrolled on the plan.


Step 4:  Phone Coaching

If you qualify for a Wellness coaching program you must complete it. If a Wellness coach is unable to reach you, you need to contact one at 800-478-1057.
