What do I have to do to request a leave?
1. Email disability@drivetime.com
or call 800-DRV-TIME, Option 1 to request a leave.
We will need to know:
• The reason for the leave (such as your own or a family
member's health condition, birth of a child, military service,
etc.)
• The first anticipated day of absence, or whether the request
is for intermittent leave.
• If the leave is due to a work-related injury we will need to
know that as well.
• DriveTime Benefits will let you know if you are eligible for
FMLA and will send you the certification packet if eligible.
2. Contact your manager regarding your need for leave and let
them know that you have been in contact with the Benefits
Department.
3. Have your doctor complete the FMLA certification packet and
fax it back to DriveTime Benefits at (866) 665-7197 within 15 days
of when the packet was sent. DriveTime Benefits will send you an
FMLA approval/denial letter based on the certification.
4. When you need to return from leave you will need to provide a
release to return to work from your doctor if the leave was due to
your own health condition. This is separate from the FMLA
certification and can be written on a prescription pad. The release
needs to be faxed to DriveTime Benefits at (866) 665-7197. You will
not be able to return to work without the release.
What if I know I'm not eligible for FMLA?
Contact DriveTime Benefits to inquire about whether you are
eligible for a Personal Leave of Absence. If you are eligible, a
request will be sent to your manager/RTM for review and you will be
notified of the decision.
How do I start my Disability claim with Lincoln if I have Short
Term Disability?
When you contact DriveTime Benefits regarding your leave request,
you will also receive information about how to start the Short Term
Disability (STD) process.
