Coverage Overview

DriveTime offers both short-term and long-term disability benefits in order to provide financial assistance if you are unable to work due to pregnancy, or a non work-related illness or injury.

DT pays for the more expensive Short term disability insurance.  The disability benefit provisions of the Short-Term Disability (STD) plan and the Long-Term Disability (LTD) plan are different. If you qualify for STD benefits, you should not presume that you will also qualify for LTD benefits.

DriveTime now offers two Long-Term Disability options. We offer our 'Core'option 1 which is our basic 5 year duration on Long-Term Disability. DriveTime also offers the 'Buy-up' option 2 which covers our employees up to the Social Security Normal Retirement Age (SSNRA) of 65. Note: either disability plan is based upon your age and salary, therefore, there will not be a flat rate for every employee.

How to Enroll

You can enroll in the DriveTime LTD plan as part of your benefit election and the coverage will be effective the 31st day following your date of hire. You are auto enrolled into STD at no cost. Please note: your New Hire window is the ONLY time you will not be subject to pre-existing conditions.

If you are on a Leave of Absence (LOA) when you request to enroll in the plan, coverage begins on the day you return to full active duty with no medical restrictions.

Definition of a Disability

You are considered disabled and eligible for benefits under the DriveTime STD plan when you are unable to perform your regular and customary work because of any physical or mental illness, injury, or condition.

Termination of Individual Coverage

Your coverage will terminate on the earliest of:

- Midnight on the date you terminate employment.
- Midnight on the date upon which you commence a LOA without pay or a layoff without pay. The date you cease to be an eligible employee. The date of death of the employee.
- The date the plan terminates; however, plan termination shall not affect any     benefits payable to you for a disability that commenced prior to plan termination.
